Freedom of Information Publication Scheme

The practice is committed to comply with the Freedom of Information Act 2000, which gives the public the right to know how public services are organised and run, how much they cost and how the decisions are made. It is the aim of the practice to ensure that members of the public have access to the identified classes of information.

Introduction
This Publication Scheme is a complete guide to the information routinely made available to the public by the dentists at the Kelvin House Dental Practice. It is not a complete list of publications since this will change as other things are produced, but it is a description of the classes of information that we provide. A full list of publications is available by contacting our Practice Manager who is responsible for our Scheme. This guide will be reviewed at regular intervals and we will monitor its effectiveness.

How much do they cost?
All information is free however there may be a charge for data sticks, CD’s or paper used if deemed necessary.

How are they published?
All of the information is available digitally or in hard copy from the practice manager. A lot of the information can also be found on our practice website.

Your right to information
As well as our published information, present and former patients of the practice have the right to access the personal information that we hold about them in accordance with the latest data protection legislation. Details are contained in the Practice Data Protection Policy that is available from the practice manager.

Classes of information
We hold various types of information that we review, retain or dispose of according to NHS rules. Our information is classed into seven categories:

1. Organisational information, structures, location and contacts
2. Financial information relating to projected and actual income and expenditure, procurement, contracts and financial audit
3. Strategies and plans, performance indicators, audits, inspections and reviews
4. Decision making processes and records of decisions
5. Current written protocols, policies and procedures for delivering our services and responsibilities
6. Currently maintained lists and registers
7. Information about the services we offer, including leaflets, guidance and newsletters produced from the public and businesses

The value of our GDS/PDS contract with the NHS and targets are available from John Barnes.
The practice provides both NHS and private treatment to patients. Information regarding the annual costs to the practice to deliver our NHS services is available from John Barnes.
